SI joint sprain, spondylolisthesis, ddd, stenosis

Hi, I have been struggling with lower back pain after a bike fall and car accident. Two years later still in pain.  I have seen physiotherapists, chiro, osteopath, massage therapist and accupuncturist.   I had an MRI and it showed a lot of things as stated in subject including  a glute med tear 1.5 cm.  I have been given So many treatment plans to no avail. Most recently a chiro diagnosed SI sprain. I attended a free worksop in Toronto and purchased Esther’sbook. I have read it and applied her techniques over the last 6 months.   I know this is a lot of information but I want to know if i should continue with 8 steps to pain free back with an SI joint sprain.  I know it takes time.

Hi Marisa,

SI joint soreness responds well to the Gokhale Method with 2 main modifications. The 1st is not to antevert the pelvis while the SI joint is sore. Stretch sitting, stretch lying on the back, rib anchor and inner corset are good places to start that do not require plevic anteversion. Inner corset can be particularly helpful.

The 2nd is to pay close attention to the feet.  Often the kidney bean shaping action is stronger on 1 side than the other, in which case focus on strengthening  the weaker foot (usually the opposite side to the sore SI joint). 

If the SI joint has been sore for a while it is likely that the ligaments at the back of the pelvis that help stabilise the SI joints may be overstretched. Wearing an SI support belt can help reduce the load on these overstretched and often inflammed ligaments.

You mention a glute med tear - difficult to give specific advice without seeing you, but if you listen to your body and let comfort be the measure (e.g. if anything seems to increase discomfort) change something or back off, dont try tp "push though pain", its there for a reason.
